@70后、80后，逐渐消失的激素，还能补回来！

随着年龄的增长，流逝的不仅是我们的青春，还有慢慢被掏空的雌激素。

道不明的失眠潮热，理不清的抑郁焦虑，我们的身体，往往比我们的心理更早认识到“我到更年期了”。虽然我们总把老母亲的唠叨、老婆日复一日的“口头教育”、领导严苛的“指责”归咎于“她更年期了”，实际上“更年期”有着自己的定义。

我们把“10个月内连续出现2次，≥7天的月经周期变化”视为进入更年期的标志。比如你原来月经28天来一次，但是变成45天来一次或者20天来一次，且是在10个月内出现2次，就算是进入更年期。而40岁以后，停经超过一年，排除怀孕就可以诊断为绝经。

拯救更年期，激素是利器

面对难捱的“更年期”，其实不需要死扛到底，适当时候可以寻求场外援助。激素，就是拯救更年期的利器之一。百闻不如一见，激素使用全攻略如下。

何时用？

根据中国绝经管理与绝经激素治疗指南（2018版），我们建议在有适应症、无禁忌症、女性本人有通过使用激素改善生活质量意愿的前提下尽早开始。

原则上不推荐＞60岁或绝经＞10年的女性开始启用激素治疗。

简单来说就是，要么不用，要用请尽早安排。

不过，尽管妇科医生一再强调，更年期熬不过去，不如试试激素，但还是有姐妹有这样那样的担心。

用激素会发胖吗？

人体内有多种多样的激素，构成了庞大复杂的体系。其中的肾上腺皮质激素，如强的松或者甲强龙的确会导致发胖。

但在围绝经期，我们一般使用的是天然或者接近天然的雌激素和孕激素，而且剂量很低，基本不会存在发胖的问题。

一项纳入了28559例女性的分析也发现，目前没有证据显示单用雌激素或雌孕激素联合治疗会增加体重[1]。所以，不要把所有的锅都丢给激素噢。

用激素会得癌吗？

也总有人认为使用激素会增加肿瘤的风险，视之如猛虎。直到现在，很多学者仍在致力研究两者关系。

子宫内膜癌：单独使用雌激素治疗会增加子宫内膜增生和子宫内膜癌的风险[2]；有子宫的女性应加用足量及足疗程的孕激素以保护子宫内膜[3]。对于Ⅰ型早期的子宫内膜癌患者，术后应用雌激素+孕激素治疗方案不增加肿瘤复发的风险[4]。

宫颈癌：国际上随机对照研究显示激素治疗不增加宫颈癌的风险[5]。

卵巢癌：研究表明，在决定采用激素治疗缓解症状时，卵巢癌不是主要考虑因素，因为激素治疗导致卵巢癌的危险度非常低（HR 1.6，95%CI 0.8-3.2；激素治疗组与安慰剂组每10万人每年分别发生42例和27例[6, 7]）。

乳腺癌：虽然关于激素治疗与乳腺癌的关系仍有争议，但目前看来，激素治疗引起的乳腺癌风险极小（＜0.1%/年），并且在治疗结束后风险会逐渐降低。乳腺癌风险增加主要与激素治疗中添加的合成孕激素及其持续应用时间有关。

目前我们多选择天然孕激素和组织选择性雌激素活性调节剂，因而风险较为可控[8]。然而，现患乳腺癌，仍然是激素治疗的禁忌症[9]。

其他恶性肿瘤：研究表明，激素治疗可降低结直肠癌[10, 11]、胃癌[12]、肝癌[13]发生风险，不增加血液系统肿瘤[14]、甲状腺癌[15]风险，但其与肺癌、胆囊癌、食管癌[16]、脑肿瘤[17]的关系仍有争议。

经验告诉我们，使用激素很有用，但也不是毫无风险，激素治疗也可能与胆囊疾病、支气管痉挛、系统性红斑狼疮、子宫肌瘤、癫痫、肾结石相关[18-22]，因此我们更需把握禁忌症与慎用症。

用了激素，啥时候停呢？

激素治疗是个体化的，医生要在综合评估患者症状、需求、风险、获益和个人期望的基础上，选择激素的种类、剂量、配伍、途径和使用时间。

不是说用了激素就可以不管不顾了，患者每年至少接受一次全面评估，并根据评估结果制定个体化的激素方案。

目前也并没有证据表明需要限制激素应用的时长，只要评估提示获益大于风险则可继续使用。

用了激素，就万事大吉了？

激素也许可以改善我们的更年期症状，让我们如获新生；但除了激素外，围绝经期全面的健康管理更为重要。

可以结合各地的饮食习惯，在全谷物纤维的基础上，

• 足量饮水（1500-1700ml/天）；

• 摄入适量蔬菜和水果、每周摄入＞2次鱼类产品；

• 烹饪时注意少油（25-30g/天）、限盐（≤6g/天）、控糖（≤50g/天）；

• 同时戒烟、限酒（酒精量≤15g/天）；

• 每日规律有氧运动，每周累计150分钟，同时进行2-3次抗阻运动。
  

衰老是人体的生理过程，卵巢功能衰退无法逆转，是千万年自然选择的最终宿命。

在出现更年期症状时，不要一熬再熬，调整心态、及时就医、健康生活、悦纳自我，才是精神不老的永恒源泉。
