弥散张量成像在脑肿瘤中的应用

[摘要]目的探讨脑肿瘤弥散张量成像(DTI)的特点及其在脑肿瘤中的诊断和鉴别诊断价值。

方法对39例经病理证实的脑肿瘤患者(脑膜瘤10例,胶质瘤17例,转移瘤12例),采用Philips

Achieva1.5T磁共振行常规MRI和DTI,在工作站上重建部分各向异性(FA)图、表观弥散系数(ADC)

图和三维白质纤维束图,选择肿瘤核心实质区和对侧镜像区为兴趣区,测量FA和ADC值,并进行t检

验。结果脑膜瘤、转移瘤和胶质瘤的FA值分别为0.36±0.08、0.28±0.03、0.18±0.06,FA值在脑膜

瘤与转移瘤、脑膜瘤与胶质瘤、转移瘤与胶质瘤之间差异均有统计学意义(P<0.05)。脑膜瘤、转移瘤

和胶质瘤的ADC值分别为1.72±0.10、1.52±0.22、1.34±0.14,ADC值在脑膜瘤与转移瘤、脑膜瘤与

胶质瘤、转移瘤与胶质瘤之间差异均有统计学意义(P<0.05)。结论DTI对脑膜瘤、转移瘤和胶质瘤

的鉴别具有很高的临床价值。
