每位年满18周岁及以上的完整填写“家庭信息表”（IMM5645），请使用中英文或中法文填写。 3 Two(2)identicalphotosofapplicantcolourorblackandwhitetakenagainstaplainwhiteorlightcolouredbackgroundwithinthelast6months.Theframesizeis35mmX45mm.Onthebackofonephotoineachset,writethenameinpinyinanddateofbirth(day/month/year)ofthepersonappearinginthephoto.
