ABSTRACT    

Objective: To investigate the local initiation role of local exosomes of acupoints in acupuncture analgesic effect. Methods: Thirty-two rats with adjuvant arthritis were randomly divided into model group (Group CFA), model + electroacupuncture group (Group EA), model + antagonist + electroacupuncture group (Group GW4869+EA), and model + dimethyl sulfoxide + electroacupuncture group (Group DMSO+EA), with 8 rats in each group. The model rat s of adjuvant arthritis were prepared by intradermal injection of 0.1 mL of Freund's adjuvant complete into the metatarsal of the right posterior foot to induce inflammation. No intervention was given in Group CFA, while electroacupuncture was performed in the other three groups at “Zúsānlĭ (足三里ST 36, bilaterally)” and “Huántiào (环跳GB 30, bilaterally)” of the rats with the following electroacupuncture parameters: dilatational wave with a frequency of 2/10 Hz, an intensity of 5/10/15 (0.76/1.53/2.3mA), a duration of 30 min, and an intensity increasing every 10 min. In Group DMSO+EA, DMSO (with a concentration of 0.2%, 50μL/acupoint) was injected at bilateral “ST 36” of the model rats one hour before electroacupuncture, while GW4869 (with a concentration of 3 mg/mL, 50μL/acupoint) was injected at bilateral ST 36 of the model rats one hour before electroacupuncture in Group GW4869+EA. The paw withdrawal latency (PWL) was used as the therapeutic effect index. 

Results: The PWL of rats in each group at Hour 24 after modeling was significantly lower than that before modeling, indicating that the models were successfully established. After the electroacupuncture treatment, the PWL of rats showed an increasing trend in all groups. The PWL of Group GW4869+EA (6.74±1.09) s was lower than that of Group EA (7.72±1.54) s on Day 1 after injection, but the difference was not statistically significant (P>0.05). The PWL values of Group GW4869+EA (7.72±0.70) s, (7.87±0.58) s were significantly lower than those of Group EA (9.96±0.94)s, (9.66±0.96) s (both P < 0.05) on Days 2 & 3 after injection, and the differences were statistically significant. 

Conclusion: It was preliminarily found that acupuncture analgesic effect was significantly reduced after local exosomes of acupoints were blocked, indicating that local exosomes of acupoints may be involved in the initiation process of acupuncture effect.

ABSTRACT IN CHINESE

 [摘要]  目的:探讨穴位局部外泌体(exosome)在针刺镇痛效应起效过程中的穴位局部启动作用。方法:将32只佐剂性关节炎(RA)大鼠随机分为模型组(CFA组)、模型+电针组(EA组),模型+拮抗剂+电针组(GW4869+EA组),模型+二甲基亚砜+电针组(DMSO+EA组)。大鼠右后足跖皮内注射弗氏完全佐剂0.1 mL致炎,制备佐剂性关节炎动物模型。其中模型组不予干预,其他三组电针针刺大鼠双侧“足三里”“ 环跳”穴,电针参数疏密波,频率2/10 Hz,强度5/10/15(0.76/1.53/2.3 mA),持续时间30 min,每隔10 min增加强度。其中DMSO+EA组电针针刺前1小时,于模型大鼠双侧“足三里”穴给予DMSO(浓度为0.2%,50μL/穴)穴位注射;GW4869+EA组电针针刺前1小时,于模型大鼠双侧“足三里”穴给予GW4869(浓度为3mg/mL,50μL/穴)穴位注射,以热辐射缩足痛阈(paw withdrawal latency,PWL)为疗效指标。结果:造模后24小时,各组大鼠热辐射缩足痛阈较造模前均明显降低,提示造模成功。电针治疗后,各组大鼠热辐射缩足痛阈均呈现上升趋势,注射第1天GW4869+EA组大鼠热辐射缩足痛阈(6.74±1.09)s较EA组(7.72±1.54)s低,但差异无统计学意义(P>0.05);注射第2、3天GW4869+EA组大鼠热辐射缩足痛阈(7.72±0.70)s、(7.87±0.58)s较EA组(9.96±0.94)s、(9.66±0.96)s显著降低(均P﹤0.05),差异存在统计学意义。结论:初步发现阻断穴位局部外泌体后针刺镇痛效应显著减弱,说明穴位局部外泌体可能参与了针刺穴位效应启动过程。

  World Journal of Acupuncture-Moxibustion (WJAM) was established in 1991, which has been administered by State Administration of Traditional Chinese Medicine of the People's Republic of China, sponsored by World Federation of Acupuncture-Moxibustion Societies (WFAS), Institute of Acupuncture and Moxibustion, China Academy of Chinese Medical Sciences (CACMS) and China Association of Acupuncture and Moxibustion (CAAM). It is an English version academic             journal from China and publish throughout the world, and the Italian version is published correspondingly.

  In 2012, WJAM was collected by Science Direct. In 2015, WJAM was collected by Chinese Science Citation Database     (CSCD). In 2016, WJAM was selected into the Emerging Sources Citation Index (ESCI) which is hold by the famous International scientific literature publishing company Clarivate Analytics (original Thomson  Reuters). The papers  of  WJAM can be searched in the subset of Web of Science Core Collection and can be searched and downloaded the same way as the journals collected by Science Citation Index (SCI). The fifth editorial board of WJAM consists of 46 renowned experts all over   the world and 58.7% of them are outside China mainland. From the beginning  of 2018, WJAM has been the full-service journal of Elsevier, and the edit and production becoming more standard and international.
