  Insufficiency fractures commonly involve the sacral ala, pubic rami, femoral necks and supra-acetabular regions. The ilium may be involved with a linear pattern similar to the more common sacral fractures. These fractures are often seen in elderly, osteoporotic females but may also occur related to radiation therapy. Fractures may be unilateral, as in this case, or bilateral. Radiographs may be normal or demonstrate areas of bone sclerosis or condensation. The fracture line may be identified, especially when the pubic rami rare involved.
