子宫内膜异位症是一种慢性妇科疾病,影响了2-22%的育龄妇女1,通常认为宫腔外存在子宫内膜样组织。它可以无症状或与不孕症、慢性盆腔疼痛和性交困难相关2。尽管其发病机制尚未完全阐明,但已知的是子宫内膜异位症是一种激素依赖型慢性炎症性疾病3,其特征是先天性免疫系统和获得性免疫系统均被激活4,5。事实上,过去20年的研究发现了大量的免疫异常:促炎细胞因子/趋化因子的产生增加,腹膜巨噬细胞浓度升高4,B细胞活化的改变,以及T/B细胞功能的免疫异常6,然而这些只是免疫功能障碍的几个例子。此外,一些参与免疫反应的基因被发现在子宫内膜异位症(III-IV期)妇女的外周血白细胞中有差异表达,与其他非妇科和慢性炎症性疾病相类似。 Shigesi及其同事最近的一项系统回顾和荟萃分析发现,患有子宫内膜异位症的女性自身免疫性疾病的共病风险增加8,包括系统性红斑狼疮(SLE)、干燥综合征(SS)、类风湿性关节炎(RA)、自身免疫性甲状腺疾病(ATD)、乳糜泻(CLD)、多发性硬化症(MS)、炎症性肠病(IBD)和Addison病,尽管纳入的研究存在与低质量相关的高偏倚风险,作者仍然报告了这一风险。事实上,子宫内膜异位症似乎具有自身免疫性疾病的共同特征,例如自身抗体的存在增加9。子宫内膜异位症患者的血清中含有高水平的抗巨噬细胞集落刺激因子抗体(抗GM-CSF抗体),这与病变的严重程度和数量相关10。GM-CSF是一种由包括子宫内膜细胞在内的许多不同细胞产生的造血生长因子,在连接先天性免疫和获得性免疫中起着至关重要的作用10,11。 自身免疫基因的一些遗传多态性也在子宫内膜异位症中进行了研究,尽管结果不一致12。这些发现使得子宫内膜异位症作为一种独特的免疫学疾病的观点成为可能,即子宫内膜异位症和免疫/自身免疫之间的可能存在某种合理联系6。然而,到目前为止,还没有研究发现子宫内膜异位症和自身免疫之间的关联是否与子宫内膜异位症的严重程度有关。 为了进一步阐明子宫内膜异位症和自身免疫之间的可能联系,我们设计了一项配对的病例对照研究,以调查合并自身免疫性疾病的存在是否与子宫内膜异位症的不同阶段有关。 结论 表1所纳入的整个队列女性的基线特征。 表2显示了患有子宫内膜异位症并伴随自身免疫性疾病的妇女(n=96)与子宫内膜异位症且自身免疫状态阴性的妇女(对照组,n=268)之间的比较,其中也显示了在人群中发现的特定的自身免疫性疾病。 两组患者在接受腹腔镜检查时的年龄(31.1±6.2vs30.10±5.4vs30.1±5.4vs30.1±5.4p=0.761)以及从接受腹腔镜检查到纳入研究的时间间隔(病例组和对照组分别为7.7士5.9vs8.0士5.7a,p=0.703)具有可比性。在纳入研究时,使用激素治疗子宫内膜异位症的女性比例在两组之间也具有可比性(病例组和对照组分别为44.8%和47.7%,p=0.617)。与不伴有自身免疫性疾病的女性相比,伴有自身免疫性疾病的女性平均ASRM评分更高(51vs39,p=0.001,图1),通常表现为IV期子宫内膜异位症的发生率(25.3%vs57.3%,p<0.001)和III期疾病的发生率(33.3%vs52.3%,p=0.001)。 在单因素Logistic分析中,伴随自身免疫(OR2.63,95%CI1.64-4.21,p<0.001)和腹腔镜手术次数(OR2.81,95%CI1.45-5.43,p 0.002)与IV期子宫内膜异位症的可能性显著相关(表3)。 在多因素Logistic回归模型中,伴随的自身免疫仍然是IV期子宫内膜异位症的一个重要预测指标(OR2.54,95%CI1.57-4.10,p=0.004),而腹腔镜手术次数与IV期子宫内膜异位症的诊断之间的关联被发现有显著意义(OR2.70,95%1.37-5.30,p=0.050,表3)。 图1.根据患者自身免疫状态进行的ASRM评分的箱形图。我们比较了没有自身免疫力的女性和伴有自身免疫性疾病的女性之间的平均ASRM评分。ASRM评分,美国生殖医学学会。 数据可获得性 在合理的要求下,本研究期间和/或本研究中产生的数据集的分析可从通讯作者处获得。 References 1.
