无论目标怎么变,以患者为中心的个体化降压治疗,才是以不变应万变。 作者|徐乃佳(主治医师) 来源 | 医学界内分泌频道 糖尿病和高血压是一对亲兄弟——与同龄人相比,高血压在2型糖尿病患者中的发病率至少是非糖尿病患者的2倍。毫无疑问,此二者共同促进糖尿病大血管及微血管并发症的发生和发展,有效控制血压是王道。近年来,相关中外指南频频问世,血压控制目标有所变更,但争议未曾间断。控制目标为啥变来变去?究竟以谁为准? 在作出回答前,有必要梳理一下国内外指南关于糖尿病患者血压控制目标的变迁历史,见表1: 表1 不同指南中糖尿病合并高血压患者血压控制目标 注: JNC:美国高血压预防检测评估与治疗联合委员会;ESH/ESC:欧洲高血压学会/欧洲心脏病学会;CHEP:加拿大高血压教育计划;ADA:美国糖尿病学会;CDS:中华医学会糖尿病学分会;AHA:美国高血压学会;ACC:美国心脏病学会 CDS推出的2017中国2型糖尿病防治指南进一步将老年糖尿病患者的血压控制目标放宽,并提出分层管理建议,见表2: 表2 2017版中国2型糖尿病防治指南老年糖尿病血压控制目标 如果患者不仅合并高血压,还兼有糖尿病肾病,部分指南推荐控制目标和仅合并高血压者一样(如表1中的JNC8、AHA/ACC、ADA、2013CDS指南)。 糖尿病合并高血压患者具有更高的心血管风险,从JNC7开始,包括我国在内的国内外多种权威指南均建议对糖尿病患者进行严格的血压控制(<130/80 mmHg),但未获得随机对照试验的支持。 直到2010年,ACCORD BP研究告诉我们,SBP控制至<120 mmHg较控制至<140 mmHg并没有显著降低心血管高危2型糖尿病患者的心血管风险;而且,强化降压组更容易出现严不良反应。与此类似,INVEST研究的糖尿病合并冠心病亚组分析显示,严格控制组(SBP<130 mmHg)与普通控制组(SBP<130-139 mmHg)主要终点无差异,同时前者的全因死亡风险增加。 荟萃分析表明,当DBP≥90 mmHg时开始降压治疗并将DBP降低到<90 mmHg,能够使患者获益。HOT研究比较了目标DBP≤90 mmHg与≤80 mmHg 的作用。糖尿病亚组较低的目标血压与心血管复合终点的减少相关,但是这是一个小亚组(8%)的事后分析,并不是预先设定的,证据的级别较低。因此,进一步降低DBP不能证明可获益。基于这些证据,以JNC8、ADA、ESH等欧美国家主要指南均将控制目标值放宽到<140/85-90 mmHg。 然而,2015年发表的SPRINT研究证实,强化降压(SBP<120 mmHg)使CVD事件风险显著降低25%,全因死亡风险显著降低27%。虽然SPRINT研究未纳入糖尿病患者,关于糖尿病患者降压目标的设定却又起波澜。深入分析ACCORD BP研究,强化降压组卒中发生率仍显著下降41%,另一项纳入73913例糖尿病患者的Mate分析也提示强化降压(<130 mmHg)使卒中风险下降39%,其它最近的Mate分析也证实强化降压确实给糖尿病患者带来更大获益。AHA/ACC在其2017年指南中又推荐严格控制糖尿病合并高血压患者的血压,亦不足为奇。CDS指南也作出调整,与之呼应。 血压控制目标,以国内最新指南为准,肯定没有错。纵观从严到松再到严的血压控制,似乎在向我们强调糖尿病患者强化降压的重要性。这不是血压目标变化的另一层含义吗? ADA在2017年和2018年糖尿病指南中均加入个体化治疗的篇幅:患者应参与血压目标的制定,制定目标时需考虑预期寿命、心血管并发症、药物不良反应(低血压等)、经济成本等多因素。 如患者具有心血管疾病高风险(尤其是卒中)、蛋白尿等因素,对强化降压控制目标耐受且无不良反应,可以强化治疗,如患者合并多种并发症、身体功能受限等,可适当放宽血压目标。CDS在2017年糖尿病指南中将血压控制目标,亦分人群设定(一般人、老年人、合并严重冠心病者)。这些事实告诉我们,无论目标怎么变,以患者为中心的个体化降压治疗,才是以不变应万变。 参考文献 1.
