近年来,糖尿病患病人数越来越多,当其因为其他疾病需要手术治疗时,围手术期的血糖控制常常让人头疼。 例如,人工关节感染(PJI)是关节置换术后的破坏性并发症之一,增加了患者的经济负担及痛苦,导致感染的因素很多;包括可控的、不可控的危险因素。Berbari EF 发表在 Clin. Infect. Dis. 的一项研究中发现:糖尿病使 PJI 风险增加 2.2 倍。 笔者就现在存在较大争议的围术期血糖控制在预防其感染中的应用进行讨论。目前对于血红蛋白 A1c(HbA1C)监测 90 天血糖控制水平的有效性存在质疑。因此,有效的血糖控制至关重要。 需要进行干预的因素:
存在的争议: 改善围手术期血糖控制确实可以改善术后效果(Namba RS,Berbari EF 相关研究表明并非所有研究都证实了糖尿病与 PJI 之间有着明确的联系)。

HbA1C HbA1C 可以反映测定前 6~8 周患者的平均血糖水平。参考值:4%~6%。 美国糖尿病学会推荐糖尿病治疗中血糖控制标准<6.67 mmol/L(120 mg/dl),HbA1C<7%;糖尿病控制不良的定义为 HbA1c ≥ 8%
果糖胺 又名:糖化血清蛋白。 NBT 法<285μmol/l,可以反映患者检测 2~3 周前的平均血糖控制水平。 患者血清白蛋白<30 g/l 或尿中蛋白质浓度>1 g/l 时检测结果不可靠。 总结
