Both images demonstrate innumerable heterogeneous T2-hyperintense foci within the bilateral kidneys.

Diagnosis:  Autosomal dominant polycystic kidney disease. 

Discussion: The cysts in this case have a heterogeneous appearance. Many exhibit fluid-bright T2 hyperintensity, others are intermediate in signal, and a few are T2 hypointense (dark). The heterogeneity reflects variable protein and blood product content within the different renal cysts.
