研究结论
专家点评 冠脉CTA虽然在排除冠心病方面表现良好,但在检测心肌缺血方面的特异性较低,限制了其临床价值。检查心肌是否缺血是指导临床决策的关键步骤,尤其是对于中度冠脉狭窄的患者。CT灌注(CTP)成像不受冠脉CTA(例如冠脉支架和弥漫性冠脉钙化)的图像质量和局限性影响。最近的一项荟萃分析[10]表明,将CTP的功能信息添加到解剖数据中,可将冠脉CTA检测限流性冠脉狭窄的特异性从64%提高到91%。 本研究证明,CTP的图像质量不受研究中心是否有操作经验所影响,并且相对MBF的临界值0.80也在先前研究的报告范围内,证明了其稳定性和一致性。与以前的双源技术相比,该研究采用的第三代双源CT的辐射剂量减少了近50%,因此,它与单光子发射层析成像研究报告的辐射剂量相当。因此,这项多中心研究结果鼓励在临床实践中更广泛地使用动态CT-MPI,尤其在弥漫性疾病患者中,CT-MPI结合冠脉CTA可以显示病变特异性的功能改变。 虽然FFR被认为是记录心肌缺血的临床参考标准,但MBF和FFR不能互换,因为它们反映冠脉生理的两个不同方面。FFR代表穿过特定心外膜冠状动脉的充血压力梯度,而MBF是心肌灌注的直接指标,同时考虑了心外膜循环和微血管阻力[11]。目前,仍缺乏将CTP与定量灌注技术(如正电子发射断层扫描和心脏磁共振)进行比较的可靠研究,这也许是未来该领域的热点研究方向。 [1] Douglas PS, Hoffmann U, Patel MR, et al.
